Care Coordinator

What will we offer you? You will be involved with our clients – both new and ongoing – through the intake period, the care planning and contractual arrangements. • Flexible work arrangements in a permanent, part-time position, working from home and with client visits. The role will commence at equivalent 1 day per week with potential to increase hours as more clients are added • A role with true purpose: you will see how you are making a difference in people’s lives • Excited and hardworking colleagues: joining a passionate and fun team You’ll be responsible for: • Co-ordinating appropriate care and services for clients in consultation with external agencies • Maintaining and updating care management systems relating to our clients and their representatives, stakeholders, service providers, and government agencies • Scheduling initial intake appointments • Sharing the scheduling of workers for clients • Liaising with clients and/or their representatives on aspects of their Home Care Package • Consulting with clients, family members, external agencies and other supports to ensure that care is ongoing and that all client needs are identified and met. • Timely follow up of incident reports and ongoing welfare checks

Skills Required

We are looking for someone with: • The roles with clients may be shared within our team, so a solid team player is a must • A sound clinical background – RN or EN preferably, but a proven capacity in hospital/medical practice administration also acceptable • An interest in and passion for aged care and community service • Practical understanding of the Aged Care Sector, Quality Standards, and Home Care Package program is desirable; a willingness to learn and comply is essential • Well organised, able to prioritise, and meet competing deadlines • Excellent attention to detail, able to problem solve and make decisions • Likely to enjoy pioneering a new role and then over time, working in a multi-disciplinary team • Good Microsoft Office skills and experience, along with a level of literacy in scheduling and reporting software if possible • The right to work in Australia / National Police Clearance or the willingness to obtain a check
