A report released by the Australian Transport Safety Bureau says it was very likely the pilot encountered meteorological conditions that resulted in spatial disorientation and a “collision with terrain”.

Investigators found weather conditions also likely led to a light plane crash that killed pilot David Stephens.

“Based on the forecast cloud, completing the flight while maintaining visual flight rules was likely not feasible.”
